This is your opportunity to weigh in on the future of Health Action! The County of Sonoma Department of Health is conducting a community-wide survey to gather perspectives on health, well-being, and equity in Sonoma County.
Health Action is a multi-sector collaborative working to implement policy and systems change strategies that foster health equity in Sonoma County. In early 2020, a group of passionate Health Action stakeholders joined forces to reimagine Health Action and bring community voice to the center of its efforts. They are in the process of redesigning Health Action now. This survey seeks to include your perspectives on what is working well, priority needs and gaps, the ideal future, and the potential role of Health Action in improving health, well-being, and equity in Sonoma County. All responses are anonymous. Insights will inform the redesign of Health Action.
